Terms of Service for HTMC Gold Membership


1. Introduction

Welcome to the HTMC Family Hub. These Terms of Service ("Terms") govern your access to and use of the HTMC Gold Membership("Membership") offered by Holy Trinity Medical Center ("HTMC," "we," "us," or "our"). By enrolling in the Membership, you agree to comply with and be bound by these Terms.


2. Definitions

"Member" or "You": An individual who has enrolled in the HTMC Gold Membership. "Services": All benefits and services provided under the HTMC Gold Membership. "Facility": HTMC's main facility and any authorized locations where services are provided." Effective Date": The date on which these Terms become effective.


3. Eligibility

3.1 Qualified Members

Membership is available exclusively to:

Active PhilHealth members and their beneficiaries. Senior Citizens with valid identification. Persons with Disabilities (PWDs) with valid identification.

3.2 Non-Eligibility

Individuals without an active PhilHealth membership are not eligible for enrollment.


4. Membership Benefits

4.1 Unlimited Scheduled Consultations

Unlimited consultations with HTMC hospitalists must be scheduled during office hours to remain free: Days: Monday to Friday; Hours: 8:00 AM to 5:00 PM

4.2 Free Diagnostic Tests

Specific free tests are offered based on age and category:

Women: Free Pap Smear

Children: Complete Blood Count (CBC)Urinalysis, Chest X-ray

Adults: Chest X-ray, Urinalysis, Blood Sugar Test, CBC

Ages 45 and Above: Basic Colon Cancer Screening + All adult services

Seniors, Diabetics, and Hypertensives: Electrocardiogram (ECG)All of the above tests

4.3 Animal Bite Center Services

Free services at our Animal Bite Center Includes a 3-dose rabies vaccine

4.4 Family Hub Doctor Availability

Access to a Family Hub Doctor (Hospitalist)Days: Monday to Friday; Hours: 8:00 AM to 8:00 PM


5. User Responsibilities and Conduct

5.1 Accurate Information

Members are responsible for providing accurate and up-to-date information during enrollment and when using our services.

5.2 Compliance with Facility Rules

Members must adhere to all HTMC facility rules, regulations, and staff instructions.

5.3 Appointment Scheduling

Members are encouraged to schedule appointments in advance and arrive on time.

5.4 Prohibited Conduct

Members shall not engage in any conduct that disrupts the provision of services or harms other patients or staff.


6. After-Hours Policy

6.1 Emergency Room Consultations

Consultations beyond regular office hours (after 5:00 PM) are considered emergency visits. A fee of ₱200 applies per after-hours Emergency Room (ER) consultation.


7. Enrollment and Registration

7.1 Direct Enrollment at PhilHealth Office

Visit the PhilHealth office located at St. Jude San Fernando. Bring a valid ID for verification. Complete the enrollment process for the HTMC Family Hub.

7.2 Single Enrollment Transfer by HTMC

We can facilitate the transfer of your registration from another facility to the HTMC Family Hub.

Important: If you are enrolled with another facility, your membership with HTMC will discontinue.

Optimal Transfer Months: November and December

Steps: Initiate a transfer request with our registration team. Provide necessary documentation. Confirm successful enrollment in the HTMC Family Hub.

7.3 Required Documentation

Active PhilHealth membership details: Valid ID for Senior Citizens or PWDs

7.4 Non-Transferability of Membership

Membership is personal to the member and cannot be transferred or assigned to another individual.


8. Fees and Payment

8.1 Membership Fee

Gold Membership Fee: Free

Membership Card Fee: A one-time fee of ₱50 for the membership card is required upon enrollment. All fees are subject to change with prior notice.

8.2 ER Consultation Fee

A fee of ₱200 applies for ER consultations beyond regular office hours.

8.3 Payment Methods

Payments can be made at our facility during office hours. Acceptable payment methods include cash and major credit cards.


9. Compliance with Philippine Healthcare Regulations

HTMC Family Hub is committed to complying with all applicable healthcare laws and regulations in the Philippines, including but not limited to:

10. Term and Termination

10.1 Term

The Membership is valid until at the end of each year.

10.2 Termination by Member

You may terminate your Membership at any time by providing written notice to us. Membership fees and card fees are non-refundable.

10.3 Termination by HTMC Family Hub

We reserve the right to terminate your Membership for violations of these Terms or misuse of services. Enrollment with Another Facility: If you enroll with another healthcare facility, your HTMC membership will automatically discontinue. You will be notified through email, call, sms or in person of the termination and the reasons thereof.

10.4 Survival of Terms

Provisions relating to Limitation of Liability, Indemnification, and other clauses intended to survive termination shall remain in effect.


11. Intellectual Property Rights

All materials, content, and trademarks on the HTMC website and within our facilities are the property of HTMC and are protected by intellectual property laws. Unauthorized use of any HTMC intellectual property is prohibited.


12. Limitation of Liability

HTMC Family Hub shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or in connection with the Membership services, to the maximum extent permitted by law. Our total liability shall not exceed the amount you have paid for the Membership Card Fee.


13. Disclaimer of Warranties

Services are provided on an "as is" and "as available" basis. HTMC disclaims all warranties of any kind, whether express or implied, including but not limited to fitness for a particular purpose.


14. Indemnification

You agree to indemnify and hold HTMC, its officers, directors, employees, and agents harmless from any claims, liabilities, damages, losses, or expenses arising out of your use of the services or violation of these Terms.


15. Force Majeure

HTMC shall not be liable for any failure to perform its obligations where such failure results from circumstances beyond our reasonable control, including but not limited to natural disasters, pandemics, or government restrictions.


16. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the Republic of the Philippines.


17. Dispute Resolution

Any disputes arising out of or relating to these Terms shall be resolved through amicable negotiation. If unresolved, disputes shall be submitted to the competent courts of San Fernando City.


18. Amendments

We reserve the right to modify these Terms at any time. Changes will be effective immediately upon posting at our facility and on our official channels. Members will be notified of significant changes via email or official notices. Your continued use of Membership services constitutes acceptance of the revised Terms.


19. Severability and Waiver

If any provision of these Terms is held to be invalid or unenforceable, such provision shall be struck, and the remaining provisions shall remain in effect. Failure to enforce any right or provision of these Terms shall not constitute a waiver of such right or provision.


20. Entire Agreement

These Terms constitute the entire agreement between you and HTMC Family Hub regarding the Membership and supersede all prior agreements.

23. Acceptance of Terms

By enrolling in the HTMC Gold Membership, you acknowledge that you have read, understood, and agree to be bound by these Terms of Service.

24. Legal Age Requirement

You represent that you are at least 18 years of age or have the consent of a parent or legal guardian to enroll in the Membership.
